紧急气管插管术辅助急诊胃镜检查抢救急性上消化道大出血7例临床分析

The Clinical Vaule of Emergency Endoscopy with Endotracheal Intubation in the Salvage of Massive Upper Gastrointestinal Hemorrhage

目的:评估紧急气管插管术下辅助急诊胃镜检查抢救上消化道大出血的临床价值。方法针对急性上消化道大出血时病人呈休克状态并易呕血误吸,无法进行急诊胃镜检查而病情急需查明病因及时抢救时,采用上述方法抢救上消化道大出血7例,均给予相应镜下治疗措施。结果本组7例病人均安全顺利通过上述方法检查,确诊了病因并进行了内镜下治疗,2例手术,6例治愈出院,抢救成功率为85.71%。1例术后24小时死于循环衰竭。结论该方法确实对急性上消化道大出血有最大可能保证了病人的安全,保持气道通畅,使病人得到及时查明病因,及时诊断和治疗,提高了抢救成功率,确实有明显的临床实用价值,在基层医院有推广的应用价值。

Objective To evaluate the clinical vaule of emergency endoscopy with endotracheal intubation in the salvage of massive upper gastrointestinal hemorrhage.Methods We chosed 7 cases.They were all hospitalized for massive upper gastrointestinal hemonhage,and were in shock at the same time.They were more likely to aspirat spiting blood Because of being in shock-state.We salvaged and treated the 7 patients through emergency endoscopy with endotracheal intubation.Results 2 of them were operated,6 of them left hospital recured,only 1 case died for circulatory failure 24 hours later.The success rates of salvage was 85.71%.Conclusion Emergency endoscopy with endotracheal intubation indeed plays important role in the salvage of massive upper gastrointestinal hemorrhage.It can keep airway opened,and ensure patients have more chose to be diagnosed and treated.This method ensures maximal safety,and elevates the rate of salvage.So emergency endoscopy w ith endotracheal intubation in the salvage of massive upper gastrointestinal hemorhage should be popularized in basic hospital.
