养阴清解法则与化瘀清解法则对照治疗非小细胞肺癌的临床研究

Clinical and Experimental Study of Treatment Based on Syndrome Differentiation by Nourishing Yin and Clearing Method for Senile Intermediate and Advanced Non-small Cell Lung Cancer

目的:主要对照观察养阴清解法则与化瘀清解法则治疗肺癌的临床疗效。方法采用单盲随机对照方法,对符合纳入标准的原发性非小细胞肺癌80例,分为治疗组(养阴清解法则组成的'肺复方组')与对照组(化瘀清解法则组成的'化瘀方组')各40例进行对照研究,1个月为1疗程,连续治疗2疗程,观察并随访记录两组临床症状,瘤体疗效、疾病进展时间,中位生存期,生存率及生活质量、体重等变化情况。结果临床研究瘤体有效率(CR+PR)治疗组15.00%,明显高于对照组7.50%(P<0.05),瘤体稳定率(CR+PR+SD)治疗组87.50%,明显高于对照组67.50%(P<0.05)。疾病进展时间(TTP)治疗组6.6月,对照组5.4月。中位生存期(MST)治疗组13.6月,对照组11.4月;1年、2年生存率治疗组分别为77.50%、55.00%,高于对照组60.00%、40.00%,(P<0.05);临床症状总有效率,治疗组87.50%高于对照组67.00%,(P<0.05);同时生活质量改善、体重改善,治疗组均优于对照组(P<0.05)。结论养阴清解法则(该法则组成的'肺复方')具有益气养阴、清热解毒之功,作为基本法则治疗肺癌,十分切合肺癌的病因病机,具有改善肺癌患者临床症状,提高生活质量,抑制、稳定瘤体,提高机体免疫功能,延缓疾病进展,延长生存期的作用均优于化瘀清解法则。提示养阴清解法则及该法则组成的肺复方是治疗肺癌的有效治则和方药。

Objective To observe the clinical therapeutic ef ects of Nourishing yin and clearing method for non-smal celllung cancer.Methods By using randomized single blind control ed method,80 cases of patient were selected according to the selecting standard and divided into experimental group and control group randomly which both had 40 cases respectively.One month a course of treatment.Clinical symptoms,tumor state,time to disease progression,median survival time,survival rate and quality of life,weight were observed and recorded in two courses of treatment.Animal experimental study on the antitumor ef ect and improve immune function was done as wel.Results Clinical study The tumor with ef iciency(CR+PR)the treatment group was 15.00%,significantly higher than control group 7.50%(P<0.05).The tumor body stable rate(CR+PR+SD)the treatment group was 87.50%,significantly higher than control group67.50%(P<0.05).TTP the treatment group was 6.6months,the control group was 5.4months.The one year and tow year survival rate were 77.50%、55.00%respectively,higher than control group 60%、40.00%(P<0.05).The total ef iciency of clinical ef ective the treatment group was 87.50%,higher than control group 67.00%(P<0.05).Experimental group’s quality of life and weight were also improved significantly.Conclusion Nourishing yin and clearing method has the function of supplementing qi and nourishing Yin,clearing heat and detoxification.As the basic rule in the treatment of lung cancer is very suitable for the etiology and pathogenesis of lung cancer.Nourishing yin and clearing method can significantly relieve the main clinical symptoms,increase the quality of life,restrain and stabilize tumor size,improve immune function,delay the progression of the disease,prolong the survival period which were confirmed in clinical and experimental study.Prompt that nourishing yin and clearing method and lung compound prescription is ef ective therapy and prescription for treatment of lung cancer.
