椎体强化术治疗骨质疏松性胸腰椎压缩性骨折

摘要 目的:探讨三种经皮椎体强化术(percutaneous vertebroplasty,PVP,bal oon kyphoplasty PKP)治疗骨质疏松性胸腰椎压缩性骨折的临床效果。方法三种方法院A组单边穿刺PVP组;B组双边穿刺PVP组;C组单边穿刺PKP组。三组均经皮椎弓根入路向椎体内穿针注入骨水泥,手术前后疼痛应用视觉模拟分级法(visual analog scale,VAS)评分。结果876例902个椎体强化均获成功,手术时间每个椎体平均35min,术后VAS(2.8±0.10)min。824例术后随访4~60个月,A组术后VAS(2.8±0.15)min;B组术后VAS(2.6±0.13)min;C组术后VAS(2.6±0.11)min;胸腰背疼痛无再发及加重,椎体高度无明显变化,相邻椎体未见骨折。结论经皮椎体强化术,操作时间短,能迅速缓解疼痛,恢复椎体高度,增加脊柱稳定性,三种方法对治疗骨质疏松性胸腰椎压缩性骨折均效果良好,但C组骨水泥渗漏率更低,更加安全。
