子宫腺肌病的治疗进展

Advances in the Treatment of Adenomyosis

子宫腺肌病治疗方法有传统的药物、手术治疗及近年来新出现的无创治疗。临床常用药物有促性腺激素释放激素激动剂、雄激素类衍生物、孕激素类和米非司酮,芳香化酶抑制剂和环氧化酶2抑制剂对子宫腺肌病的疗效正在探讨中。手术治疗包括根治手术和保守手术。根治手术即为子宫切除术,保守手术包括腺肌病病灶切除术、子宫内膜及肌层切除术、盆腔去神经支配治疗、腹腔镜下子宫动脉阻断术、子宫腺肌病的血管介入性治疗。无创治疗为超声或磁共振引导下高强度超声手术等。

Treatments for adenomyosis are traditional medical therapy,surgical therapy and noninvasive treatment in recent years.Clinical commonly used medicines as gonadotropin-releasing hormone agonist,androgen derivatives,progesterone hormones and mifepristone;the ef icacy of aromatase inhibitors and cyclooxygenase 2 inhibitors for adenomyosis is being studied now.Surgical treatments for adenomyosis includes radical surgery and conservative surgery.The radical surgery is hysterectomy.The conservative surgery includes adenomyomectomy,resection of endometrium,pelvicdenervation therapy,uterine arterial blockage,uterine arterial embolization.Noninvasive treatment is ultrasound or magnetic resonance guided high intensity focused ultrasound.
