全膝关节置换术后早期膝前痛相关因素的研究

摘要 目的通过探讨全膝关节置换术后髌股关节解剖关系与早期膝前痛的相关性,了解术后早期膝前痛的相关因素。方法2009年2月~2012年8月回顾性研究241例全膝关节置换术。术后早期存在行走时膝前痛的患者31例31膝入选实验组,男8例8膝,女23例23膝,年龄(66.3±6.02)岁。术后不存在早期行走时膝前痛的患者30例30膝入选对照组,男9例9膝,女21例21膝,年龄(66.1±6.59)岁。随访时间为6个月,分别记录术后1月、3月、6月X片上股骨滑车角、外侧髌股角、髌股和谐角及髌股指数等多个参数,并作比较研究。结果无论在术后1月、3月还是6月,实验组与对照组在X线片上的股骨滑车角、外侧髌股角比较上并无显著性差异(>0.05),而在髌股和谐角及髌股指数比较时存在显著性差异(<0.05)。结论全膝关节置换术后髌股和谐角及髌股指数两个解剖学参数可能与术后疼痛有关,而股骨滑车角、外侧髌股角可能与术后疼痛无关。
