椎间盘镜微创手术治疗腰椎间盘突出症的临床疗效观察

Clinical Efficacy of Microendoscopy Discetomy for the Treatment of Lumbar Disc Herniation

目的:观察椎间盘镜微创手术治疗腰椎间盘突出症的临床疗效。方法选取2010年10月~2012年10月,68例明确诊断为单节段腰椎间盘突出症的患者,按就诊先后顺序随机分为治疗组和对照组,其中治疗组34例,行后路椎间盘镜下髓核摘除术;对照组34例,行传统开放单纯髓核摘除术。分别比较两组切口长度,手术时间长短,术后住院日数和手术出血量,两组病例围手术期切口疼痛评分(VisualAnalogScale,VAS),术后3个月功能障碍指数(ODI)以评估症状改善程度。结果治疗组与对照组相比,其切口小、手术时间短、术后住院日数少、手术出血量少、术后切口疼痛轻,但近期ODI改善程度相近。结论椎间盘镜微创手术治疗腰椎间盘突出症与传统的开放手术相比,其具有切口小、手术时间短、出血量少、恢复快、手术后疼痛轻等优势,是一种安全、有效的治疗方法。

Objective To observe the clinical ef icacy of microendoscopy discetomy for the treatment of lumbar disc herniation.Methods From 2010 October to 2012 October,68 cases of single segmental lumbar disc herniation were treated in our department.They were randomly divided into two groups according to the dif erent operation method.In the 34 patiensts suf ered from only one lumbar disc herniation opearted by MED compared with the ame disease,samples operated by traditional opened operaiton.The length of incision,operation time,postoperative hospital stay,blood loss,perioperation period Visual Analog Scale(VAS)preoperative and postoperative March Oswestry Disability Index(ODI)to asses the degree of the symptom improvement were compared between two groups.Results Compared with the treatment group and the control group,the treatment group has smal er incision,shorter operation time,less postoperative hospitalization stay,the operation less bleeding and postoperative incision pain,but the ODI of recent improvement is similar.Conclusion Microendoscopy discetomy for the treatment of lumbar disc herniation has smal er incision,shorter operation time,less blood,faster recovery,less postoperative pain and other advantages than the traditional open surgery.Microendoscopy discetomy surgery is minimal y invasive,safe and effective method for the treatment of lumbar disc herniation.
