摘要
目的 研究新疆帕金森病(PD)发病风险与血尿酸(UA)、甘油三脂(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)之间的关系,并进一步探讨PD与UA之间是否存在严重程度及临床分型之间的差异.方法 采用病例-对照研究,收集新疆地区123例PD患者作为病例组,在同一时期收集健康对照组人群95例,其性别、年龄均与病例组相匹配.比较分析两组UA、血脂水平的差异性.同时依照Hoehn-Yahr(H-Y)分期PD患者按疾病的严重程度分为早期、中期、晚期3个亚组,依照UPDRS评分将PD患者分为震颤型(TD)、姿势不稳和步态障碍型(PIGD)、中间型(I)3个亚组,分别比较分析各组间UA水平有无差异.结果 UA、TG、TC、LDL-C、HDL-C明显低于对照组,差异有统计学意义(UA t=4.179,P<0.001;TG t=3.217,P=0.001;TC t=2.853,P=0.005;LDL-C t=2.567,P=0.011;HDL-C t=2.287,P=0.023),不同H-Y分期的PD患者UA水平差异有统计学意义,中期和晚期的PD患者UA水平低于早期PD患者,晚期PD患者UA水平低于中期PD患者(F=9.729,P<0.001);不同临床分型的PD患者UA水平差异有统计学意义,PIGD患者UA水平低于TD患者和I患者,I患者尿酸低于TD患者(F=20.402,P<0.001).多因素Logistic回归分析结果显示UA、HDL-C与PD的发病风险有关,高尿酸、高HDL-C为保护因素.结论 血UA、TG、TC、LDL-C、HDL-C可能与帕金森病的发病风险有相关性.
Objective To analyze the correlation between blood uric acid(UA),triglyceride(TG),total cholesterol(TC),low density lipoprotein-cholesterol(LDL-C),high density lipoprotein-cholesterol(HDL-C)and the risk of Parkinson's disease(PD),and to explore whether there were correlation of severity or subtype of PD and UA.Method It was a case-control study.123 patients with PD in Xinjiang were collected as experimental group.95 healthy people with same ages and sex were chosen as control group.The differences of UA and blood lipid between two groups were compared.According to the Hoehn-Yahr(H-Y)staging,123 PD patients were divided into early stage,intermediate stage and late stage subgroups.According to the UPDRS scores,123 PD patients could be divided into three subgroups:tremor-dominant(TD),postural instability and gait disorder(PIGD),and intermediate type(I).The differences in UA between the groups were also compared.Results UA,TG,TC,LDL-C,HDL-C in experimental group were significantly lower than those in the control group(UA t=4.179,P<0.001;TG t=3.217,P=0.001;TC t=2.853,P=0.005;LDL-C t=2.567,P=0.011;HDL-C t=2.287,P=0.023).The UA values in intermediate and late stage were lower than that in early stage;UA in late stage was lower than that in intermediate stage.(F=9.729,P<0.001).And UA in PIGD patients was lower than that in TD and type I patients;UA in type I patients was lower than that in TD patients(F=20.402,P<0.001).Multivariate logistic regression analysis showed that decreased UA and HDL-C were correlated with risk of PD.Conclusion UA,TG,TC,LDL-C,HDL-C may be associated with the risk of Parkinson's disease.
