子宫内膜射频消融治疗更年期功血50例疗效观察

Observation of endometrium radiofrequency ablation in the treatment of climacteric dysfunctional uterine bleeding with 50 cases

目的 研究子宫内膜射频消融治疗更年期功血的疗效.方法 选取2016年1月~2018年12月我院收治的更年期功血患者100例作为研究对象,将其以随机数表法均分为观察组和对照组.对照组予以宫腔镜子宫内膜电切术治疗,观察组则予以宫腔镜子宫内膜射频消融术治疗.分别比较两组疗效、各项手术指标水平、术后并发症发生情况以及手术前后性激素相关指标水平变化情况.结果 观察组治疗总有效率为94.00%(47/50),高于对照组的78.00%(39/50),差异有统计学意义(P<0.05).观察组手术时间、术中出血量以及住院时间少于对照组,差异有统计学意义(P<0.05).观察组术后并发症发生率为2.00%(1/50),低于对照组的14.00%(7/50),差异有统计学意义(P<0.05).术前两组FSH、E2水平比较,差异无统计学意义(P>0.05),术后6个月两组FSH水平较术前升高,E2水平较术前降低,差异有统计学意义(P<0.05),而术后6个月观察组FSH水平较对照组降低,E2水平较对照组升高,差异有统计学意义(P<0.05).结论 宫腔镜子宫内膜射频消融术治疗更年期功血的疗效明显,有利于促进患者早日康复,安全性较高,对患者的性激素水平影响较少.

Objective To study the efficacy of endometrial radiofrequency ablation in the treatment of climacteric dysfunctional uterine bleeding.Methods 100 patient with climacteric dysfunctional uterine bleeding who were treated in our hospital from January 2016 to December 2018 were selected as research subjects,and they were divided into observation group and control group by random number table method.The control group was treated with hysteroscopy endometriosis resection,and the observation group was treated with hysteroscopy endometrium radiofrequency ablation.The clinical efficacy,operative indexes,postoperative complications and changes of sex hormone levels before and after operation were compared between the two groups.Results The total effective rate of the observation group was 94.00%(47/50),which was higher than 78.00%(39/50)of the control group(P<0.05).The operative time,intraoperative blood loss and hospitalization time in the observation group were less than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was 2.00%(1/50),which was lower than 14.00%(7/50)of the control group(P<0.05).There was no significant difference in the levels of FSH and E2 between the two groups before operation(P>0.05).At 6 months after operation,the level of FSH in the two groups was higher than that before operation,and the E2 level was lower than that before operation(P<0.05).At 6 months after operation,the level of FSH in the observation group was lower than that in the control group,and the level of E2 was higher than that in the control group(P<0.05).Conclusion Hysteroscopic endometrial radiofrequency ablation is effective in the treatment of climacteric dysfunctional uterine bleeding.It is beneficial to promote the early recovery of patients,it has higher safety,and it has little effect on the sex hormone levels of the patients.
