逆针灸贴脐法对季节性过敏性鼻炎的防治作用研究

Effect of preventive treatment of seasonal allergic rhinitis with umbilical therapy based on preventive acupuncture

目的 观察逆针灸贴脐法对季节性过敏性鼻炎的临床防治效果.方法 将102例临床确诊为季节性过敏性鼻炎的患者以随机数字表法分为春秋组、秋季组和对照组,每组各34例,最终纳入统计分析的患者共93例(秋季组脱落2例,对照组脱落7例).贴脐药物以桂枝汤合玉屏风散、苍耳散组成.春秋组分别于立春前4周和立秋前4周贴脐,秋季组仅在立秋前4周贴脐,对照组则无任何干预措施.各组在干预前与干预后均进行鼻症状积分(TNSS)与鼻结膜炎生活质量调查问卷(RQLQ)评分,进行临床疗效评定.结果 与干预前和对照组比较,春秋组及秋季组经干预后TNSS及RQLQ总分下降(P<0.05);春秋组在立春前干预后的疗效与秋季组比较,差异无统计学意义(P>0.05);春秋组立秋前干预后的总有效(70.59%)高于本组立春前干预(58.82%)及秋季组(53.13%);长病程的有效率(27.78%)低于短病程的有效率(75.00%);不同证候的总有效率依次为肺气虚寒证(69.70%),肾阳不足证(46.67%),脾气虚弱证(38.89%).结论 逆针灸贴脐法能有效防治季节性过敏性鼻炎,改善患者的临床症状,提高生活质量,疗效与治疗次数、病程长短及证候类型相关,为该病的防治提供新思路.

Objective To observe the clinical preventive effects of umbilical therapy based on preventive acupuncture on seasonal allergic rhinitis.Methods 102 patients with seasonal allergic rhinitis were divided into three groups with random number table:spring-autumn treatment group,autumn treatment group and control group,34 cases in each group,and 93 cases were included in the final statistical analysis(the autumn treatment group lost 2 cases and the control group lost 7).The spring-autumn treatment group received umbilical therapy consisting prescriptions of Guizhi(Cinnamon Twig)Decoction,Yupingfeng(Jade Screen)Powder and Canger(Xanthium)Powder for four weeks before the Beginning of Spring(the day marking the beginning of the 1st solar term according to Chinese lunar calendar,usually on Feb.3,4 or 5)and the Beginning of Autumn(the day marking the beginning of the 13th solar term,usually on Aug.7,8,or 9).The autumn treatment group received treatment only for four weeks before the Beginning of Autumn,while><385B>the Control Group received no intervention.Patients were scored according to Total Nasal Symptom Score(TNSS)and Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)before and after treatment for clinical efficacy evaluation.Results Compared with the the scores before treatment and those of the control group,the total scores of TNSS and RQLQ respectively in the two treatment groups decreased after preventive treatment(P<0.05).There was no significant difference in the curative effect between the spring-autumn treatment group after the spring treatment and the autumn treatment group after the autumn treatment(P>0.05).The total effective rate of the springautumn treatment group after the autumn treatment(70.59%)was higher than that of the same group after the spring treatment(58.82%)and that of the autumn treatment group after the autumn treatment(53.13%).The effective rate of patients with a long course of disease(27.78%)was lower than that of those with a short course(75.00%).The total effective rate for different patterns were 69.70%for deficiency-cold of lung-qi,46.67%for insufficiency of kidney-yang and 38.89%for weakness of spleenqi respectively.Conclusion The umbilical therapy based on preventive acupuncture can prevent seasonal allergic rhinitis effectively,and improve the clinical symptoms and life quality of patients significantly.Its efficacy is related to the number of treatments,the course of disease and the type of pattern according to Chinese medicine.
