横断疝囊对腹腔镜疝修补术治疗GilbertⅢ型腹股沟疝的疗效分析

Analysis of therapeutic effect of laparoscopic hernia repair with transverse hernia sac for Gilbert type Ⅲ inguinal hernia

目的探讨横断疝囊对腹腔镜疝修补术治疗GilbertⅢ型腹股沟疝的疗效。方法回顾性分析2019年1月至2022年1月如皋市中医医院普外科收治的经腹腔镜腹股沟疝修补术治疗的单侧GilbertⅢ型腹股沟疝患者60例的临床资料。按疝囊处理方式分为横断疝囊组30例和完全剥离疝囊组30例,分析比较2组患者的一般资料、手术相关指标(手术方式、手术时间、术中失血量、术后住院天数)、术后早期疼痛评分、术后并发症(术后血清肿、尿潴留,慢性疼痛)及疝复发情况。结果2组患者在一般资料、手术相关指标、术后早期疼痛评分、并发症方面,差异均无统计学意义(P>0.05)。2组患者随访1年均无复发。结论腹腔镜疝修补术中采用横断疝囊的方式治疗GilbertⅢ型腹股沟疝同完全剥离疝囊的方式比较疗效无明显差异,2种疝囊处理方式的优劣差异有待进一步研究。

Objective To explore the effect of transverse hernia sac on laparoscopic hernia repair for Gilbert typeⅢinguinal hernia.Methods The clinical data of 60 patients with unilateral Gilbert typeⅢinguinal hernia treated by laparoscopic inguinal hernia repair in General Surgery Department of Rugao Hospital of Traditional Chinese Medicine from January 2019 to January 2022 were retrospectively analyzed.According to the management of hernia sac,patients were divided into the transverse hernia sac group and the complete dissection hernia sac group,with 30 cases in each group.General data,surgical indicators(surgical method,operation time,intraoperative blood loss,postoperative hospital stay),early postoperative pain score,postoperative complications(seroma,urinary retention,chronic pain)and recurrence of the two groups were analyzed and compared.Results There was no significant difference between the two groups in terms of general data,surgical indicators,early postoperative pain score,and complications(P>0.05).There was no recurrence in the two groups after one year of follow-up.Conclusion In laparoscopic hernia repair,there is no significant difference in the treatment of type GilbertⅢinguinal hernia between transverse hernia sac and complete dissection hernia sac.The advantages and disadvantages of the two hernia sac treatments need to be further studied.
