脱白细胞温氧合血灌注体外循环术患者早期血清人心脂肪酸结合蛋白的变化

Changes of serum level of HH-FABP in the early postoperative period of extracorporeal circulation with warm oxygenated leukocyte-depleted blood cardioplegia perfusion

目的:了解脱白细胞温氧合血灌注体外循环术患者早期血清人心脂肪酸结合蛋白(HH-FABP)的变化。方法:选择主动脉阻断时间>40min的体外循环手术20例,随机分为观察组和对照组,每组10例。观察组用脱白细胞温氧合血进行灌注,对照组不滤除灌注液中的白细胞。于主动脉阻断前(术前)、主动脉阻断30min、主动脉开放即刻、20min、40min、60min、90min,抽取2组患者右房血3mL,ELISA法检测血清HH-FABP水平。结果:主动脉开放40～60min,2组血清HH-FABP均达峰值。主动脉阻断前、阻断30min、开放即刻,2组血清HH-FABP水平差异无统计学意义(P均>0.05),而在主动脉开放20min、40min、60min、90min时,观察组血清HH-FABP水平低于对照组(P<0.05)。结论:脱白细胞温氧合血灌注可减轻心肌损伤,有利于心肌的保护。
