摘要
目的了解特发性血小板减少性紫癜((idiopathic thrombocytopenic purura,ITP)患者的免疫功能并探讨ITP的发病机制。方法应用流式细胞术检测41例ITP患者及18例正常对照者外周血淋巴细胞亚群;同时与患者的临床资料进行相关性分析。结果(1)ITP患者外周血淋巴细胞亚群的表达中,CD19+的表达率明显增加(P<0.01);CD3+、CD3+CD4+、CD3+CD4+/CD3+CD8+的比值、CD3-CD16+CD56+(NK细胞)的表达率明显降低(P<0.05)。(2)患者外周血CD19+的表达率与患者发病至就诊时间呈正相关(r=0.50,P=0.02),与患者骨髓巨核细胞数值均呈正相关(r=0.62,P=0.002);患者CD3+CD4+的表达率与患者发病至就诊时间呈负相关(r=-0.49,P=0.02);患者CD3-CD16+CD56+的表达率与患者巨核细胞数值呈负相关(r=-0.43,P=0.04)。结论ITP患者细胞免疫及体液免疫功能的紊乱可能与ITP的发病有关;ITP患者NK细胞的异常表达可能是ITP的致病机制之一。
