斜行套叠再造胃底His角治疗贲门癌

Treatment of gastric cardia cancer with oblique intussusceptive His recycling angle

目的探讨贲门癌手术中斜行套叠再造胃底His角的抗反流作用及远期效果。方法对162例贲门癌病人手术中采用斜行套叠再造胃底His角的胃食管吻合方式进行消化道重建。结果全部病人于术后第6天常规行X线钡餐透视检查,头低足高位无钡剂反流。随访6个月至5年,12例夜间经常反流,自述反流液苦味明显,伴有胸骨后烧灼感。此12例病人行胃镜检查,见吻合口及下段食管炎症明显,但无明显溃疡形成。上消化道钡餐透视检查均发现十二指肠钡剂瘀滞,十二指肠明显扩张。结论斜行套叠再造胃底His角的胃食管吻合方式,在正常生理状态下具有较好的抗反流作用。但部分病人还应注意解决十二指肠的排空。
