二维及彩色多普勒超声、彩色多普勒能量图在宫外孕诊断中的应用价值

Clinical value of two-dimensional imaging,CDFI and CDE in the diagnosis of ectopic pregnancy

【目的】探讨二维、彩色多普勒(CDFI)、彩色多普勒能量图(CDE)对比动态监测在宫外孕中的应用价值。【方法】对319例宫外孕患者进行经腹部与阴道二维及CDFI、CDE对比观察,观察宫腔、附件及盆腔情况,对超早宫外孕及不典型宫外孕进行近日内动态监测。【结果】可清晰显示孕囊与胎芽与原始心搏动86例(26.96%),破裂型95例(29.78%),不典型型138例(43.26%);二维超声诊断准确率78.8%,CDFI91.7%,CDE 98.9%。【结论】二维超声可对宫外孕疑似患者筛选检查,对不典型宫外孕近日内复查,动态监测,必要时可经阴道CDFI、CDE检查,监测附件区混合性包块内及周边的血流信号及流速指数,并结合临床病史、月经史、阴道不规则流血史及尿HCG史和血-βHCG试验,对宫外孕的早期诊断与鉴别诊断和及时治疗有重要价值。

【Objective】To discuss the clinical value of two-dimensional imaging,CDFI and CDE in the diagnosis of ectopic pregnancy.【Methods】319 cases of ectopic pregnancy were studied by abdominal and trans-vaginal two-dimensional imaging,CDFI and CDE on the uterine cavity,adnexa,fallopian tube and pelvic cavity.In case of very early and non-classic ectopic pregnancy dynamic observation was performed.【Results】Blast cyct,blastema and primitive cardiac pulse were clearly observed in 86 cases(26.96%),rupture was found in 95 cases(29.78%) and atypical type was found in 138 cases(43.26%).【Conclusion】Two-dimensional imaging can be used in case of ectopic pregnancy for screening,in case of atypical type,dynamic observation is indicated.Vaginal CDFI and CDE to detect the fluency and rate of blood are also indicated if necessary.Clinical history,menstrual history,history of irregular vaginal bleeding and related examination is valuable for diagnosis.
