2型糖尿病患者糖尿病性肾病不同阶段血脂谱的差异 被引量:1

摘要 目的:比较2型糖尿病患者糖尿病肾病不同阶段的血脂谱。方法:收集我院2型糖尿病患者144例,根据尿蛋白定量、尿微量白蛋白定量分成三组:无肾病组22例,早期肾病组78例,临床期肾病组44例。所有患者测定血甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A(apoA)、载脂蛋白B(apoB)、脂蛋白a。比较三组间血脂差异。结果:HDL-C水平在无肾病组、早期肾病组、临床期肾病组依次递减(1.363±0.535,1.161±0.327,1.060±0.281),三组间两两比较,差异均有显著性。TG水平依次递增(1.359±0.485,1.581±0.827,1.963±1.626),其中临床期肾病组较早期肾病组及无肾病组增高差异有显著性,早期肾病组与无肾病组间差异无显著性。脂蛋白a水平呈递增趋势(9.252±2.920,11.368±5.954,12.333±6.354),但组间差异无显著性。3组间TC、LDL-C、apoA、apoB无差异。结论:2型糖尿病患者脂代谢紊乱与糖尿病肾病的发生有一定相关性。 Objective:To investigate the difference of serum lipid profile in different stages of nephropathy in type2 diabetes mellitus. Methods:144 type2 diabetic patients were collected during Jan 2005 to March 2006,and then divided into three stages of nephropathy accrodding to urinary protein excretion rate and urinary albumin excretion rate, 22 non-nephropathy, 78 earlystage nephropathy,44 clinical-stage nephropathy.Total triglyceride(TG),cholesterol(TC),high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol(LDL-C),and apolipoproteins A (ApoA) and B (ApoB),and apolipoprotein(a) were measured in the fasting state for all patients.Results:HDL-C level was lower in earlystage nephropathy compared to non-nephropathy,lower in clinicalstage nephropathy compared to earlystage nephropathy.On the contrary,serum triglyceride was higher in clinicalstage nephropathy compared to non-nephropathy and earlystage nephropathy.Conclusion:Dyslipidemia is associated with nephropathy in type2 diabetic patients.
