脑内海绵状血管瘤的MRI诊断

Diagnosis of cerebral cavernous angioma using MRI

目的探讨脑海绵状血管瘤的MRI表现及诊断价值。方法回顾性分析我院收治的42例脑海绵状血管瘤患者的影像学表现。全部患者行MRT1WI、T2WI序列扫描,11例增强扫描。结果41例共有52个病灶,均呈结节状或类圆形,1例7个病灶呈散在分布圆点形。38个病灶T1WI、T2WI均呈高、低混合信号,3个病灶呈T1WI稍低信号,T2WI高信号,5个病灶T1WI、T2WI均呈高信号,此46个病灶T2WI均见黑色低信号环影。6个病灶T1WI、T2WI均呈低信号,1例7个病灶T1WI、T2WI均呈低信号。结论脑海绵状血管瘤T1WI、T2WI显示信号多样,但具有特征性。MRI对多数脑海绵状血管瘤能明确诊断。

Objective To probe manifestations of cerebral cavernous angioma on MRI image as well as their diagnostic value. Methods 42 patients with cerebral cavernous angioma were respectively reviewed for their MRI data. All patients had undergone MR T1WI and T2WI sequence, and 11 received enhanced scan. Results There were 52 lesions in 41 patients displaying nodular or round-alike shape. One patient had 7 scattering lesions resembling round spots. 38 lesions had a mixture of hypo- to hyperintensity, 3 lesions showed slightly hypointensity on T1WI and hyperintensity on T2WI, 5 lesions had hyperintensity either on T1WI or T2WI, all 46 lesions had black hypointensity ring on T2WI. 6 lesions had hypointensity on both T1WI and T2WI, and 7 lesions of 1 patient had hypointensity on both T1WI and T2WI. Conclusion Cerebral cavernous angioma manifests a wide variety of intensities on T1WI and T2WI, and imaging features are characteristic. MRI is able to diagnose most of cerebral cavernous angioma.
