多层螺旋CT在乳腺良性疾病中的应用

Application of multislice spiral CT to diagnosing benign breast lesions

目的评价多层螺旋CT(MSCT)及后处理技术在乳腺良性疾病诊断中的应用价值。方法回顾性分析我院2002年9月～2003年7月收治的23例经手术病理确诊为乳腺良性疾病的术前CT资料。患者行MSCT平扫及增强扫描,图像行MPR、MIP、SSD后处理。结果23例乳腺良性疾病中,乳腺纤维腺瘤12例,CT平扫7例呈圆形密度较高肿块,5例呈轻度分叶状肿块,增强扫描表现为不均匀强化,有5例中心无强化;乳腺导管扩张并囊肿形成8例,CT平扫见乳腺内圆形低密度肿块,CT值<20Hu,边缘光滑,5例周边可见较高密度囊壁;乳腺炎2例,CT表现不规则形块影,密度不均匀,增强后强化不均匀,皮肤增厚;乳腺脂肪瘤1例,CT表现为圆形肿块,边缘光滑,密度均匀,CT值-80Hu,增强后轻度强化。结论MPR、MIP、SSD后处理技术对乳腺良性疾病的显示各有优势。MSCT能够较准确地诊断乳腺良性疾病的部位和性质。

Objective To investigate value of Multisided Spiral CT (MSCT) and imaging reconstruction in diagnosis of benign breast lesions. Methods 23 patients were admitted by our hospital during September 2002-July 2003 and they were con- firmed benign breast diseases by surgical pathological examinations. Before operation, the patients underwent MSCT plain and enhanced scan and reconstruction treatment including MPR, MIP and SSD. Imaging data of the 23 patients were retrospectively analyzed. Results Among 23 patients, 12 were adenofibroma, 7 were round mass displaying hyperintensity on CT plain scan, 5 were slightly lobulated and became unevenly enhanced on enhanced scan, 5 patients were not enhance in the center, 8 patients had dilated duct of mammary glands accompanied by formation of cyst, presenting round hypointensity mass in the mammary glands whose CT value was <20 Hu and edge was smooth; 5 patients had thickened cyst walls surrounding the lesions; 2 patients were mastitis displaying irregular masses, uneven density, uneven enhancement and thickened skin; one patient had lipoma in mammary glands displaying round mass with smooth edge and even density. The CT value was -80 Hu and could be slightly enhanced. Conclusion Reconstructions as MPR, MIP and SSD have their own uniqueness helping diagnose benign breast diseases. MSCT can accurately display location and nature of the benign breast lesions.
