他汀类药物对2型糖尿病及其合并大血管病变患者血浆NO及EET水平影响的研究

Effects of Statin Treatment on EETs and NO in Type 2 Diabetes with or without Macro-vascular Complications

摘要 目的观察2型糖尿病及其大血管病变患者经他汀类药物短期治疗前后血浆NO和EET的变化情况,评估其对2型糖尿病及其大血管并发症患者的保护作用。方法28例2型糖尿病患者,男女各14例,口服辛伐他汀40 mg/d治疗2周,治疗前后分别检测血浆NO和EET水平、CRP、外周血单个核细胞计数。结果(1)大血管病变组年龄、糖尿病病程、空腹血糖、血肌酐、24 h尿微量白蛋白、收缩压、血浆炎症相关指标CRP、外周血单个核细胞计数明显高于无大血管病变组,血浆NO、EET水平明显低于无大血管病变组;(2)2型糖尿病及其大血管并发症患者经辛伐他汀短期治疗后血浆NO、EET明显升高;CRP水平、外周血单个核细胞计数明显降低;(3)两组2型糖尿病患者经辛伐他汀短期治疗后血浆EET、NO升高程度与血浆炎症相关指标CRP降低程度及外周血单个核细胞计数降低程度呈正相关。结论2型糖尿病患者血浆CRP水平、外周血单个核细胞计数明显增高,处于炎症状态。辛伐他汀短期治疗能显著减低2型糖尿病患者血浆CRP水平、外周血单个核细胞计数,上调血浆NO、EET水平。这为2型糖尿病患者使用辛伐他汀预防心脑血管疾病提供临床证据。 Objective To observe the change in plasma level of endothelium-derived dilating factors nitric oxide(NO)and epoxyeicosatrienoic acids(EETs)in type 2 diabetic patients before and after statin treatment and evaluate the protective effect on type 2 diabetic patients with or without macro-vascular compli- cations.Methods Two-week-long therapy of Statin for the selected two groups of Type 2 diabetic patients (total 28 persons),we extract 10 ml peripheral vein blood from subject before and after Statin therapy.We separate peripheral blood mononuclear cells(PBMC)and distill nucleoprotein,then detect the activity of NF-(B P65.After that we determine the level of plasma NO,EET,CRP and leucocyte count.Results (1)The age,during,fasting blood glucose,creatinine,24h-urine micro-albumin,systolic pressure signifi- cantly higher than the group without macro-vascular complications.(2)The plasma NO and EET level in two type 2 diabetes groups were generally increased after stain treatment;the CRP level and the number of pe- ripheral blood mononuclear cells of patients were generally decreased.Conclusion The CRP level and leu- cocyte count were markedly increased in type 2 diabetes,which mean these patients were in the inflammatory state.The plasma NO and EET level were generally increased,and the plasma CRP level,leucocyte count were generally decreased after statin treatment.This preliminary data from diabetic patients provide an in- sight into relation between diabetes and endothelial function as well as a scientific foundation for diabetic pa- tients to take statin for the sake of preventing eardio-cerebral vascular diseases.
