摘要
目的 探讨宫颈癌及癌前病变中金属硫蛋白(MT)的表达及其与细胞增殖、凋亡的关系.方法 应用免疫组化SP法检测不同病变宫颈组织中MT和增殖细胞核抗原(PCNA)的表达,应用TUNEL法检测原位细胞凋亡.结果 在正常宫颈、慢性宫颈炎、宫颈上皮内瘤样变(CIN)Ⅰ~Ⅱ级、CINⅢ级、宫颈浸润癌中,MT阳性表达率和增殖指数(PI)均依次递增(均P<0.05);在CINⅠ~Ⅱ级、CINⅢ级、宫颈浸润癌中,凋亡指数(AI)逐渐降低(均P<0.05),但明显高于正常宫颈和慢性宫颈炎;MT表达阳性组PI较高,AI较低,与MT阴性组比较,差异有显著性意义(均P<0.05);MT的表达与宫颈癌的病理类型和组织学分级密切相关(均P<0.05),与临床分期和淋巴结转移无关.结论 MT可能通过促进细胞增殖和抑制细胞凋亡参与了宫颈癌的发生,检测MT的表达对宫颈癌的早期诊断及恶性程度的评价具有重要参考价值.
Objective To study the relationship between the expression of metallothionein(MT) and cell proliferation and apoptosis in cervical cancers.Methods Immunohistochemical SP method was used to detect the expression of MT and PCNA in different cervical tissues,and cell apoptosis was detected by TUNEL method.Results From normal cervical epithelia,chronic cervicitis,cervical intraepithelial neoplasia(CIN)Ⅰ-Ⅱ,CINⅢ to cervical cancers,both the MT expression and PI were gradually increased(all P<0.05).The apoptosis indexes(AI) in CIN Ⅰ-Ⅱ,CINⅢ,and cervical cancers were gradually diminished(P<0.05),but higher than those in normal cervix and chronic cervicitis.The proliferating index(PI) in MT positive cases was higher than in negative ones,while the AI was lower(P<0.05).In cervical carcinoma the MT expression was significantly related to the pathologic type and histological grade(P<0.05),but not to the clinical stage and lymph node metastasis(P>0.05).Conclusion MT may participate in the carcinogenesis of cervical carcinoma by enhancing the cell proliferation and suppressing apoptosis.The detection of MT expression is important for early diagnosis and malignancy evaluation of cervical cancer.
