Clinicopathological Implication of O^(6)-methylguanine-DNA-methyltransferase Gene Expression in Gastric Carcinoma

摘要 目的检测胃癌组织O6-甲基鸟嘌呤-DNA-甲基转移酶(MGMT)基因的表达,探讨MGMT基因表达与胃癌组织学类型、疗效、预后、骨髓抑制的关系。方法应用加强型原位杂交三相寡核苷酸探针高效标记技术检测76例胃癌组织MGMT的表达,同时应用自动图像分析系统进行定量分析。结果76例胃癌MGMT基因阳性表达率为40.8%(31/76)。治疗后生存5年以上者胃癌组织MGMT阳性表达率为35.3%(4/17),明显低于生存5年以下组,后者MGMT阳性表达率为65.9%(22/41)(P<0.05)。白细胞计数≥3×109/L组MGMT阳性率为54.8%(23/42),明显高于白细胞计数<3×109/L组[23.5%(8/34)](P<0.01)。早期胃癌与进展期胃癌间MGMT表达差异无显著性意义(P>0.05)。不同组织学类型及淋巴结转移状况组间MGMT基因表达差异无显著性意义(P>0.05)。结论胃癌MGMT基因阳性表达水平与患者生存时间、骨髓抑制有关,提示检测胃癌组织MGMT基因表达水平对抗癌治疗、提高骨髓耐受性和预测预后有重要意义。 Objective To detect the expression of O^6-methy1guanine-DNA-methyltransferase(MGMT) in human gastric carcinoma tissue and study the possible relationship among MGMT expression and histological types,curative effectiveness,prognosis,bone marrow inhibition in gastric carcinoma.Methods The MGMT gene expression in 76 cases of gastric carcinoma was detected by using enhanced sensitive situ hybridization with high sensitive digoxygenin labeled oligonucleotide probe technique,and quantitatively analyzed by auto-imaging analysis system.Results The MGMT positive expression rate in 76 cases of gastric carcinoma was 40.8%.In the patients with survival time more than 5 years,the MGMT positive expression rate was 35.3%,significantly lower than that in the patients with survival time less than 5 years(65.9%)(P<0.05).In the group of white blood cells(WBC) ≥3×10^(9)/L,the MGMT positive expression rate was 54.8%,significantly higher than that in the group of <3×10^(9)/L(23.5%,P<0.01).There was no significant difference in the MGMT expression between early-stage gastric carcinoma and advanced gastric carcinoma(P>0.05).Also,there was no significant difference in the expressionof MGMT gene among different histological types and lymph node metastases(P>0.05).Conclusion The expression level of MGMT gene was related with survival time and bone marrow inhibition in the patients with gastric carcinoma,suggesting that detection of MGMT gene in gastric carcinoma tissue may play an important role in the therapy of gastric carcinoma and improving tolerance of bone marrow and predicting prognosis.
