通心络胶囊联合氯沙坦治疗高血压肾损害的研究

Study of Tongxinluo capsule combined with Losartan on kidney damage caused by hypertension

目的 观察通心络胶囊联合氯沙坦治疗高血压肾损害的临床疗效。方法 将 5 2例患者单盲随机分为 2组 ,2组均予接受氯沙坦 5 0mg口服 ,每天 1次 ,如治疗 2周仍不能达目标血压 (130 / 80mmHg ,1mmHg =0 .133kPa) ,则增加氯沙坦用量至 10 0mg每天 1次 ;治疗组在原有治疗基础上加服通心络胶囊 ,每次 4粒 ,每日 3次。达目标血压后 ,测定内生肌酐清除率(Ccr)、尿白蛋白排出率 (UAE)、血尿 β2 -微球蛋白 (β2 -MG)、尿N -乙酰 - β-氨基葡萄糖苷酶(NAG酶 )。 2组持续治疗 8周后再次测定上述项目。结果 治疗前后 2组UAE、Ccr比较有显著性差异 (P <0 .0 1) ;治疗后 2组UAE、Ccr、尿 β2 -MG、NAG酶比较有显著性差异 (P <0 .0 5 ) ,治疗组治疗前后尿β2 -MG、NAG酶比较有显著性差异 (P <0 .0 5 )。结论 通心络胶囊与氯沙坦联合应用在延缓高血压肾损害进展方面能够起到协同作用。

Objective It is to observe the clinical curative effect of Tongxinluo capsule combined with Losartan on kidney damage caused by hypertension. Methods 52 patients were randomly divided into treatment group and control group and all were given Losartan 50mg taken orally once a day. Increased 50 mg of Losartan dosage each day if it can't reach the target blood pressure (130/80 mmHg, 1 mmHg=0.133 kPa). Treatment group were given Tongxinluo capsule four grains a time and three times a day based on those given. The creatinine clearance (Ccr), urine albumin excretion (UAE) rate, blood and urine β 2-microgloculin (β 2-MG) and urinary N-acetyl-β-glucosaminidase (NAG enzyme) were measured after it reached the target blood pressure. Those items above were measured again eight weeks after treatment. Results There were significant differences in UAE and Ccr of the two groups between before and after treatment (P<0.01). There were significant differences in UAE, Ccr, urine β 2-MG and NAG enzyme between the two groups after treatment (P<0.05). There were significant differences in urine β 2-MG, NAG enzyme in treatment group between before and after treatment (P<0 05). Conclusion The combination application of Tongxinluo capsule and Losartan has a synergistic action on delaying the kidney damage caused by hypertension.
