食管癌放疗的研究进展

Development and study of radiotherapy for esophageal carcinoma

目的:综述了近几年食管癌由常规二维放疗向三维适形放疗的发展演变及两者的比较分析。方法:以食管癌、放疗和三维适形为检索关键词,采用万方数据库和pubmed中英文数据库进行检索,尤其是2004~2007年发表的相关文献,共收集文献169篇,通过查找全文,排除重复或与本研究无关的文献,选取具有代表性的重点文献27篇,纳入分析。结果:随着近年来三维适形放疗(3D-CRT)的出现,国内对其研究也逐渐增多,尤其是3D-CRT与常规放疗在定位、剂量分布及临床疗效等方面的比较已成为国内的研究热点。诸多学者认为,常规模拟定位容易遗漏靶区,CT定位靶区勾画精确,更利于放疗计划设计及靶区剂量分布。结论:3D-CRT计划通过精确定位、精确计划及精确治疗,使靶区剂量提高,分布均匀,周围正常组织如肺、脊髓受量降低,进而提高了食管癌放射治疗的疗效和生存率,并减少了正常组织的不良反应。
