摘要
目的比较腹腔镜和开腹结直肠癌手术对机体应激反应的影响。方法选择该院自2004年12月～2006年2月收治的结直肠癌患者40例。分为腹腔镜组和开腹组各20例。比较两组围手术期血糖、胰岛素、皮质醇、醛固酮及相关临床指标的变化。结果两组患者性别、年龄、体重指数、Dukes分期及手术方式差异无统计学意义(P>0.05)。两组患者术后第1天血糖、皮质醇明显升高,胰岛素水平明显降低,腹腔镜组于术后第2天恢复到术前水平,开腹组于术后第3天恢复至术前水平。两组患者术后第1天醛固酮较术前有升高,但差异无统计学意义(P>0.05)。腹腔镜手术时间超过5h时(6例)血糖、皮质醇和胰岛素的变化与开腹手术相当,均于第3天恢复正常。结论腹腔镜结直肠癌手术后应激反应持续时间短,强度低。腹腔镜手术时间超过5h,应激持续时间及强度与开腹手术相当。
[Objective] To investigate the difference of systemic stress responses in patients receiving laparoscopic colectomy and laparocolectomy. [Methods] 40 patients with colorectal cancer in our department from Dec. 2004 to Feb. 2006 were selected, and were equally divided into 2 groups (laparoscopic group and laparotomy group).
