前路多椎体次全切除植骨融合治疗多节段颈椎后纵韧带骨化症

Treating ossification of the posterior longitudinal ligament by anterior multi vertebral subtotal vertebrectomy and bone graft fusion

目的分析前路多椎体次全切除植骨融合术治疗多节段颈椎后纵韧带骨化症(OPLL)的效果。方法多节段连续型或混合型OPLL患者14例,经前路结合磨钻技术开"烧瓶"状骨槽,直视下切除骨化的后纵韧带,减压后牵引下植骨并安置钢板,观察术后植骨融合和并发症发生情况,并按JOA评分和X线摄片,判定改善率和优良率。结果14例患者获3～20个月(中位时间10个月)的随访。无1例并发神经根麻痹、脊髓和椎动脉损伤等并发症,4个月后均获骨性融合。按JOA评分,优8例(57.1%),良4例(28.6%),好转2例(14.3%)。结论结合磨钻技术前路多椎体次全切除术是治疗多节段颈椎后纵韧带骨化症一种较好的方法。
