摘要
目的了解急性肾衰竭(ARF)在重症医学科(ICU)病房的病因分布和预后情况。方法收集2002年10月-2007年1月我院ICU病房(ARF)病例74例,统计基础病因、治疗手段、转归情况的差异。结果我院ICU病房急性肾衰竭(ARF)患病率4.74%,病因构成以严重感染占总ARF(35.1%)为首,其次依次是大手术占总ARF(32.4%)严重创伤占总ARF(16.2%)重症胰腺炎占总ARF(10.8%)。而病死率比较最高为重症胰腺炎62.5%,严重感染病死率(53.8%)略高于大手术和严重创伤病死率(50.0%)。分析ARF和并多器官衰竭28天病死率为单纯ARF的死亡率为25%,和并器官衰竭数1个、2个、≥3个脏器衰竭的死亡率分别为45.5%、68%、81.8%。且四组的病死率统计学差异有显著性(P<0.01)。比较74例(ARF)病例治疗方法,保守治疗(40例)死亡数28例死亡率(70%),CRRT治疗(34例)死亡数14例死亡率(41.1%)。且统计学差异有显著性(P<0.01)结论对于ICU病房患者应积极预防ARF的发生,及时进行CRRT治疗,减少MODS发生,降低死亡率。
Objective In order to know the etiological factors and pre-wards situations of acute renal failure (ARF) in intensive care unit (ICU). Methods Collect 74 cases of ARF in ICU of our hospital from October 2002 to January 2007.
