腰椎间盘突出症并椎管狭窄症的手术治疗观察

The operative treatment of the lumbar intervertebral disc herniatan with lumbar vertebral vessel stenosis

目的探讨腰椎间盘突出症合并椎管狭窄症手术治疗方法及疗效分析。方法 5年间治疗106例,采用开窗38例,半椎板切除57例,全椎板切除11例,术中应用环锯开窗、钻取髓核,保留棘突,棘间韧带、棘上韧带的连续性,自体脂肪片覆盖硬膜囊。结果平均随访时间2年8个月,优73例,良28例,可5侧,差者无,优良率为95.3%。结论严格掌握术式适应证定位准确,减压彻底是手术成功的关键,应用环锯开窗、钻取髓核安全、快捷、彻底,不易复发。

Objective To investigate the operative treatment of the Lumbar intervertebral disc herniation and lumbar vertebral Vessel Stenosis and to analyse its dinial effects.Methods 106 cases were treated during 5 years,in- eluding 38 Windowing 57cutting half the vertebral bar 11 Cutting the Whole Vertebral bar in the operation, We applized windowing with the circenate and acpuired the bone marrow with the dirll (Reeping the spines, interspinal ligaments as well as the ligaments on the spines intact),and then autotransplanted the fat flake to cover the membrane cysidesl vesicle.Results 106 cases were followed up with an average period of 32 monthes The excellent results were obtained in 73 cases、good in 28、fair in 5 and poor in 0.The excellent and good cases tookup 95.3% .Conclusions Strictly controlling the operative indications accurate location and thoroughly reducing the oppression are the crucial factors for a successful operaton.Appliing windowing with the circinate saw and acquiring the marrow with the drill are sara time-savlng,thorough and have the stable effects。
