左旋门冬酰氨酶致下肢静脉血栓一例附文献复习

摘要 左旋门冬酰氨酶是急性淋巴细胞性白血病治疗用药,它可使凝血系统发生改变而出现众所周知的血栓或出血性并发症,该文报道一例47岁男性病人在左旋门冬酰氨酶应用后致下肢静脉血栓的的病例。该病人在经过10d左旋门冬酰氨酶输注后出现右胫前静脉血栓,APTT92.8s,PT16.7s,抗凝血酶3(AT3)58%,D-二聚体(D-DI)增高为1.96μg/mL(0.01～0.50μg/mL),纤维蛋白原2.25g/L,PLT128×109/L;新鲜冰冻血浆输注未能纠正凝血系统异常。因APTT和PT显著延长亦未予溶栓及抗凝治疗。1周后患者栓塞部位肿胀已消褪,疼痛较前明显好转。 Objective Changes in the coagulation system due to asparaginase during treatment for acute lymphoblastic leukemia (ALL) are well known side effects and may cause bleeding or thrombosis.We report the case history of a 47-year old male patient who developed thrombosis of the right lower limb during ALL-treatment.
