宫腔镜子宫内膜电切术对卵巢功能远期影响的研究

Long-term influence of transcervical resection of endometrium on ovarian function

目的探讨子宫内膜电切除术对卵巢功能的远期影响。方法选择45岁以下因子宫良性病变行子宫内膜电切除术的患者(TCRE组)及同期行子宫切除术的患者(子宫切除组)各50例,于手术前、术后6个月、术后1年、术后2年检测血清FSH、LH、E2、P、PRL、T水平,经阴道彩色多普勒超声测定卵巢动脉血流阻力指数(RI)、搏动指数(PI)和卵巢大小,进行更年期症状评分,测定基础体温。结果TCRE组术前与术后各时期6项性激素水平、PI、RI、卵巢前后径、左右径、更年期评分、双向体温比例组内比较,差异无显著性。子宫切除组术后6个月、术后1年、术后2年与术前组内比较及与TCRE组行组间比较E2、P、TSH、LH差异有显著性。子宫切除组术后E2、P逐渐下降,TSH、LH逐渐升高。经阴道彩色多普勒超声测定子宫切除组术后6个月、术后1年、术后2年PI、RI升高,术后1年、术后2年卵巢前后径、左右径减小,与术前组内比较及与TCRE组行组间比较差异有显著性。子宫切除组术后1年、术后2年更年期评分升高,双向体温比例下降,与术前组内比较及与TCRE组行组间比较差异有显著性。结论该研究在观察期间内子宫内膜电切除术对卵巢功能无影响,子宫切除术对卵巢功能有一定影响。

[Objective] To evaluate the long-term influence of transcervical resection of endometrium (TCRE) on ovarian function. [Methods] The patients were 50 cases, under 45-year-old, who underwent transcervical resection of endometrium and the other 50 cases who underwent hysterectomy with ovarian reserved during the same period because of the uterus benign lesion.
