基于信息技术平台支撑下的社区与综合医院双向转诊新模式研究

New Model of Dual Referral between Community and General Hospitals Basing on Information Technique

目的基于信息技术平台,合理配置区域医疗资源,创新双向转诊模式,实现对社区居民个人、家庭全方位、生命全周期、全人群的健康管理。方法以北京大学人民医院为综合医院和两家社区卫生服务中心作为试点单位,建立基于信息技术平台支撑下的双向转诊模式。结果社区卫生服务中心与综合医院建立了综合医院及社区信息交换平台、管理及业务组织网络,制定了双向转诊的流程和标准及急诊转诊的绿色通道、收费标准、监督及制约机制,实现了实验室检查数据的共享。上转有效率为95.6%,下转有效率为100.0%。结论社区卫生服务中心与综合医院建立了基于信息平台支撑下的双向转诊的机制,为开展及完善医疗改革政策提供科学依据。

Objective Basing on information technique platform,to reasonably distribute the resources of regional medical resource and create dual referral model so as to realize the health management of the whole life of both individual,family and al people.Methods The people′s hospital of Beijing University and two community health service centers were taken as the pilot to set up a dual referral model basing on the information technique platform.Results A information exchange platform between the community health service centers and the general hospital and the network of management and professional construction were set up.A dual referral circuit and standards,as well as green passage for emergency referral,charge standard,managing and restraining mechanism were formulated,and resource sharing of laboratory test data was achieved.The effective rate upward referral was 95.6%,and that downward referral was 100.0%.Conclusion Between the community health service centers and general hospital a dual referral system based on information technique is established so that it provides a science basis for medical reform.
