Foley氏尿管在心内直视手术中治疗动脉导管未闭的应用

Use of Foley catheter in therapy of Patent Ductus Arterius with open cardicac surgery

目的 :探讨在心内直视手术中以Foley氏尿管阻断动脉导管血流 ,缝闭动脉导管的方法。方法 :在浅低温、心脏不停跳、并行循环下切开主肺动脉 ,将Foley氏尿管经动脉导管插入主动脉 ,然后向尿管球囊内注入适量生理盐水 ,轻轻提拉尿管 ,阻断动脉导管血流后 ,行动脉导管闭合术。结果 :全组 5 2例应用Foley氏尿管控制动脉导管处分流的血液后 ,术野清晰 ,手术操作方便可靠。平均住院日 16 .5天 ,全部病人康复出院 ,除开展此项技术早期发生一例灌注肺外 ,无重大并发症发生。结论 :应用Foley氏尿管阻断动脉导管血流 ,有利于术野显露 ,缩短体外循环时间 ,有利于心肺保护 ,同时避免组织撕裂 ,喉返神经损伤和残余分流等并发症。

Objective:To discuss occulation of pulmonary artery blood flow with Foley catheter in open cardiac surgery.Methods:Cut the pulmonary artery with mild hypothermic,heart beating circulation,Insert the Foley catheter into aorta through PDA and inject NS to the balloon.Undergo PDA closare after blood flow occlccsion.Results:52 cases with Foley catheter in operation to control bleecling,and make the operation yard clear.All of the patients discharged which arerage hospitalizing time are 16.5 dags.No complication happened except one case with punop lungs.Conclusion:Occlusion of PDA blood flow with Foley catherer makes operation yard clear,shorten operation time benifit to cardiac and pulmonary protection and prerent complications.
