动脉栓塞化疗在四肢恶性骨肿瘤保肢术前的应用

Application of TACE in osteosarcoma before conserved extremity operation

目的:研究四肢恶性骨肿瘤保肢手术前经导管动脉栓塞化疗(Transcatheterarteritalchemobolization,TACE)的临床应用价值。方法:回顾性分析经手术及病理证实的12例骨肉瘤,分别采用了碘油悬浊液及明胶海绵行超选择性动脉栓塞化疗;术后平均2～6天进行保肢手术。结果:经介入治疗后的全部病例有11例瘤体被完全切除并保留肢体,7例疼痛症状明显减轻;3例疼痛症状基本消失;全部病例均有不同程度的功能改善,软组织肿块变小或消失,术中大大减少了出血量。介入后肿瘤染色及碘油沉积与病理坏死结果显示一致,4例显示轻中度坏死(20%～60%);7例显示重度坏死(60%～100%)。结论:经导管动脉内栓塞化疗术是四肢骨肉瘤保肢术前的一种十分有效的方法,值得应用推广。
