地氟醚、异氟醚与氧化亚氮复合麻醉对心率变异性影响的比较研究

摘要 目的:应用地氟醚、异氟醚与氧化亚氮复合麻醉对冠心病及冠心病高危因素病人心率变异性(heart rate variability,HRV)进行比较,并探讨其对心脏自主神经系统的影响.方法:美国麻醉医师学会(ASA)分级II~III级择期腹部手术病人30例,地氟醚组14例,异氟醚组16例,术前已明确诊断冠心病或具备冠心病高危因素.比较诱导前15min(基础值)、诱导后5min、呼气末麻醉气体浓度为0.5,1.0,1.5MAC,停吸入麻醉药后10,30,60,120,180min时的HRV.结果:两组的总功率(total power,TP)、低频功率(low frequency,LF)、高频功率(high frequency,HF)于诱导后均较术前下降,组间无显著性差异(P>0.05).地氟醚组于停药后30min,异氟醚组至停药后180minTP,LF,HF恢复至基础值水平.两组LF/HF在1.5MAC时明显低于术前,其余时段与基础值无显著差异.结论:地氟醚、异氟醚与氧化亚氮复合麻醉对冠心病及冠心病高危因素的病人心脏自主神经系统产生程度相似的抑制作用.地氟醚麻醉后心脏自主神经系统活性恢复的速度快于异氟醚.
