摘要
研制无创性食管曲张静脉测压仪并探讨其可行性及临床应用价值。方法 食管曲张静脉压力测定仪由气路部分、气敏探头、电路部分、数据处理部分和压力控制手柄组成。采用配对设计 ,研究体外实验和动物实验静脉穿刺测压值与贴壁测压值的直线相关关系。临床研究 2 0例食管静脉曲张患者食管曲张静脉贴壁测压值与食管曲张静脉的静脉压力值的直线相关关系。结果 体外实验和动物实验研究穿刺测压值与无创性贴壁测压值有直线相关关系 (r =0 .99,P <0 .0 0 1)。临床研究食管曲张静脉贴壁测压值与穿刺测压值有直线相关关系 (r =0 .97,P <0 .0 0 1)。结论 无创性食管曲张静脉测压仪能准确测定食管曲张静脉内压 。
Objective The noninvasive endoscopic esophageal variceal pressure measurement device is modified and tested for its feasibility and validity.Methods The device consists of air infusion system, pressure sensor gauge,fixing apparatus, the signal transducer and data processing system. The device have been tested for its feasibility in vitro and in vivo models and performed clinical trials in 20 patients with esophageal varices for its validity.Results The study in vitro and in vivo which was conducted on dog models showed excellent correlation between the noninvasive device gauge measured pressure and the invasive needle puncture measured pressures(r=0.99,P<0.001),the clinical trial performed in 20 patients also showed excellent correlation between the noninvasive and the invasive measured pressures(r=0.97,P<0.001).Conclusion The results suggest that the esophageal varices could be measured accurately by the device and the noninvasive measured esophageal variceal pressure is valid in clinical evaluation of patients with varices at risk of abrupt bleeding.
