51例听神经瘤显微手术切除和面神经保留方法分析

Analysis of Preservation of the Facial Nerve During Acoustic Neuroma Microsurgery(A report of 51 cases)

目的 :探讨听神经瘤显微切除术和面神经保留技术。方法 :5 1例听神经瘤均采用枕下乙状窦后入路 ,显微手术切除肿瘤 ,并尽可能做到面神经解剖保留。结果 :肿瘤全切除 45例 (88.2 % ) ,次全切除 6例 (11.8% ) ,面神经解剖保留 44例 (86 .3% ) ,7例 (13 .7% )未保留成功。结论

Objective:To investigate the surgical technique of preservation of the facial nerve during acoustic neuroma surgery.Methods:Fifty one cases of acoustic neuroma underwent tumor removal microsurgically by suboccipital approach,and the anatomic integrity of the facial nerves were kept as complete as possible.Results:Total tumor removal was achieved in 45 of the 51 patients(88.2%),subtotal in 6(11.8%).The facial nerves were kept anatomic intact in 44(86.3%) of the patients ,the others were not.Conclusion:Identifing the original segment of the facial nerves and its relationship with the tumor,Separating the tumor sharply near internal acoustic meatus are the key to kept the anatomic integrity of the facial nerve.
