抗细胞间粘附分子-1单克隆抗体对心肌缺血再灌注损伤的保护效应 被引量:1

摘要 应用大鼠心肌缺血再灌注模型 (缺血 45min ,再灌注 2 4h) ,观察抗细胞间粘附分子 1(ICAM 1)单抗对心肌坏死面积、左室收缩和舒张功能以及中性粒细胞浸润的影响。结果表明 ,实验组心肌梗死范围 (19 2 5 %±5 6 1% )与手术对照组 (31 71%± 6 46 % )差异显著 (P <0 0 5 ) ;实验组左心室收缩期压力峰值 (LVSP)、心室等容收缩期室内压上升的最大速率 (+P′max)、左心室舒张末期压 (LVEDP)、心室内压最大下降速率 (-P′max)与对照组相比差异显著 (P <0 0 5 )。研究还发现实验组大鼠心肌坏死区中性粒细胞浸润数显著低于对照组 (P <0 0 5 )。提示抗ICAM 1单抗能减轻心肌缺血再灌注损伤 ,改善心肌收缩和舒张功能 ,这种保护作用可持续到再灌注 2 4h后。 The present study was designed to evaluate the effect of anti intercellular adhesion molecule 1(ICAM 1) monoclonal antibody in a rat model of myocardial ischemia reperfusion injury (I/R). Anaesthetized rats were subjected to total occlusion (45 min) of the left main coronary artery followed by 24 h reperfusion. The area of myocardial necrosis, myocardial function (such as LVSP, +P′max, LVEDP and -P′max) and PMN infiltration were measured. We found that the area of myocardial necrosis in treated rats was smaller than that in control group ( P <0 05). The value of LVSP and +Pmax in treated rats were significantly increased compared with that in untreated rats. Improvement of myocardial relaxation function was also observed in treated rats. LVEDP and -Pmax in treated group were significantly different from that in untreated group. The amount of PMN infiltration in the necrotic area in treated group was significantly decreased compared with untreated group ( P <0 05). These data indicated that anti ICAM 1 monoclonal antibody may behave as a protective agent to have a potential clinical value in the prevention and treatment of myocardial ischemia reperfusion injury.
