两种不同形状可折叠人工晶状体植入的临床对比观察

摘要 目的观察超声乳化白内障术后植入两种不同设计形状的可折叠人工晶状体对后囊膜混浊(posterior capsuleopacification,PCO)以及后囊皱褶(posterior capsule striae)的影响,以及两者间的关系。方法回顾性分析2006年9月至2008年3月在我院进行年龄相关性白内障手术的患者88例(109眼),按植入的人工晶状体类型分为两组:A组(博士伦公司LI61SE型)43例(55眼),B组(博士伦公司AkreosAdapt人工晶状体)45例(54眼)。术后平均随访5个月,记录其矫正视力,并散瞳观察其后囊中央及周边混浊、皱褶情况。结果植入LI61SE和Akreos Adapt可折叠人工晶状体均可获得较好的视力,最佳矫正视力分别为0.76±0.19和0.74±0.18,两组间差异无统计学意义(t=0.690,P>0.05)。后囊中央部混浊发生率:A组为21.82%,B组为7.41%;周边部皱褶发生率:A组为23.63%,B组为9.26%;A组发生率均大于B组,差异有统计学意义(χ2=4.518,χ2=4.085,P<0.05)。人工晶状体后囊周边部混浊发生率:A组为36.36%,B组为35.19%;中央部皱褶发生率:A组为9.09%,B组为5.55%;两组差异均无统计学意义(χ2=0.016,χ2=0.116,P>0.05)。A组后囊中央及周边混浊伴随皱褶发生率具有相关性(Kappa值分别为0.393和0.618,P<0.05);B组后囊中央及周边混浊伴随皱褶发生率无明显相关性(Kappa值分别为0.068和0.121,P>0.05)。结论超声乳化白内障术中植入不同形状的可折叠人工晶状体,术后其晶状体后囊中央部混浊发生率、周边部皱褶发生率有差异,植入不同形状可折叠人工晶状体后发生的后囊皱褶现象与后囊混浊现象相关性有差异。 Objective To compare the differences in posterior capsule opacification and posterior capsule striae after phacoemulsi-fication when foldable intraocular lenses with two different shapes are implanted.Methods Clinical data and follow-up results were retrospectively analyzed from 88 patients(109 eyes),who were treated in the Department of Ophthalmology,Daping Hospital,The Third Military Medical University,from September 2006 to March 2008.Fifty-five eyes(A group)of 43 patients were implanted with the LI61SE ...
