血浆同型半胱氨酸与脑卒中关系的病例对照研究

Case-control study on the relationship between plasma homocysteine level and stroke

目的探讨血浆同型半胱氨酸(Hcy)水平与脑卒中之间的关系。方法收集136例脑卒中患者为病例组,选择136例与病例组相匹配的无脑卒中者作为对照组,采用荧光偏振免疫分析法测定血浆Hcy,采用化学发光法测定叶酸和维生素B12,并将结果进行比较。结果血浆Hcy水平:脑出血组为:(18.98±9.92)μmol/L;脑梗死组:(20.36±8.78)μmol/L;均高于对照组:(13.23±1.89)μmol/L,比较差异有显著性(P<0.01)。叶酸水平:脑出血组:(4.89±5.25)ng/mL;脑梗死组:(4.94±5.61)ng/mL;低于对照组:(7.92±2.16)ng/mL,比较差异有显著性(P<0.01)。维生素B12水平:脑出血组:(316.25±189.43)pg/mL;脑梗死组:(310.25±163.89)pg/mL;对照组:(426.75±203.88)pg/mL,比较差异有显著性(P<0.01)。结论高Hcy血症是脑卒中的独立危险因素。血浆Hcy水平与叶酸、维生素B12呈负性关系。
