摘要
目的探讨乳管镜和乳腺定位导丝对乳管内肿瘤的诊断和定位价值。方法2006年7月至今,对28例Ⅰ、Ⅱ级导管内肿瘤患者行乳管镜检明确肿瘤及位置,经乳管镜置入乳腺定位导丝,确认其倒钩将肿块钩住。术中选择乳晕边缘小切口,沿定位导丝切除乳头下至距肿瘤远端2cm病变乳管及瘤体。剖开乳管定位瘤体后送病理检查。结果28例患者中发现早期乳腺癌2例(7.1%),乳管内乳头状瘤20例(71.4%),乳头状瘤病6例(21.4%)。结论乳管内镜能直接观察乳头溢液患者乳管内情况及肿块,提高乳头溢液患者病因诊断的准确性,而在其协助下经溢液乳孔放置乳腺定位导丝,能够避免手术盲目性,为病灶的检出提供可靠的帮助。
