胃切除术后胆结石形成的分析及处理

Clinical Analysis and Treatment of Cholelithiasis after Gastrectomy

目的探讨胃切除术后并发胆道结石的可能形成机制及处理方法。方法随访胃大部切除术后和胃癌根治术后存活至今的病例,回顾性分析术后胆结石的发病及处理情况。结果胃大部切除术后患者52例,10例(19.2%)出现胆结石;胃癌根治术后患者36例,15例(41.7%)出现胆结石。两组胆结石发病率均显著高于自然人群的6.5%(P<0.01)。进行淋巴结清扫的胃癌根治术患者术后胆石症的发生率高于未清扫淋巴结的胃大部切除术患者(P<0.05)。微创手术处理胃切除术后胆结石患者术后第3d可出院,开腹手术处理后患者1周后出院。结论胃切除术后胆结石发病率较高,胆道动力学改变在结石的发生中起着重要作用。微创手术处理胃切除术后胆石症具有优越性。
