不同年龄发病的系统性红斑狼疮的临床分析

Clinical Analysis of Systemic Lupus Erythematosus with Different Onset

目的:探讨不同年龄发病的系统性红斑狼疮的实验室指标及临床特点的异同。方法:回顾性分析189例不同年龄发病的系统性红斑狼疮的临床及实验室资料。结果:ANA、dsDNA抗体在少年及青年发病组显著高于中老年发病组(P<0.05);C3、IgG、IgM在青年发病组高于少年和中老年发病组,但仅较中老年发病组有统计学意义(P<0.05);血液系统、肾脏损害在少年及青年发病组发生率显著高于中老年发病组(P<0.05,P<0.01),中老年发病组肌肉及关节受累发生率高于前二组(P均<0.01);中老年发病组重症SLE的发生率明显低于少年和青年发病组(P均<0.01)。结论:与中老年发病组相比,少年和青年发病组的SLE患者的免疫指标异常及血液系统、肾脏受累更常见,重症SLE发生率更高,提示少年及青年发病的SLE患者体内有更强的自身免疫应答,中老年发病的SLE患者的病情侵袭性更低。
