摘要
目的:研究临床中分离的耐亚胺培南铜绿假单胞菌的产金属酶的检测及耐药现状,为合理选择抗生素提供理论依据。方法:采用EDTA纸片复合法和E-test法分别筛选产金属β-内酰胺酶菌株,同时用PCR法检测blaIMP-1和blaVIM-2耐药基因,用MIC法测定产金属β-内酰胺酶菌株对8种常用抗菌药物的敏感度。结果:30株耐亚胺培南铜绿假单胞菌ED-TA纸片复合法筛选9株产金属酶菌株;E-test法筛选6株产金属酶菌株;PCR法仅检测到1株blaIMP-1和3株blaVIM-2耐药基因。产金属酶菌株呈多重耐药,尤其是PCR法产金属酶菌株耐药呈现为高水平耐药;阿米卡星抗菌活性最好,其次为环丙沙星。结论:金属酶是铜绿假单胞菌对亚胺培南耐药的原因之一,我院临床分离铜绿假单胞菌中部分携带金属酶,该酶导致该菌对碳青霉烯类耐药。
