摘要
目的为观察芬太尼鞘内注射用于预防剖宫产术麻醉后寒战的临床效能。方法选择ASAⅠ~Ⅱ级足月妊娠初产妇60例,随机分为两组,每组各30例。芬太尼组(F组):0.75%布比卡因1.5 mL+芬太尼25μg(0.5 mL)+注射用水1.0 mL。对照组(B组):0.75%布比卡因1.5 mL+注射用水1.5 mL。观察术中产妇寒战的发生情况,并连续监测呼吸和循环功能,观察围手术期血流动力学变化及不良反应的发生,评估麻醉效能。结果两组患者寒战发生率F组(2例)与B组(27例)比较差异有统计学意义(P<0.05)。感觉阻滞起效时间、最大感觉阻滞平面、平面固定时间、运动阻滞起效时间的差异,均无统计学意义(P>0.05)。不良反应:发生恶心、呕吐F组9例,B组3例;皮肤瘙痒F组7例,B组0例,差异均有统计学意义。结论芬太尼鞘内注射,可以有效预防产妇腰麻后寒战的发生,但不良反应发生率相应增加。
