摘要
目的观察三维彩色多普勒超声卵巢恶性肿瘤的血管成像特点,探讨其对卵巢恶性肿瘤的临床应用价值。方法二维模式下观察32例38枚卵巢囊实性肿块的大小、形态、边界、内部回声以及肿块内血流情况。启动CDFI显示肿瘤区域血管分布特征,停帧测量阻力指数(RI),RI<0.4作为诊断恶性肿瘤的界值,3D模式下进行血管三维重建,将诊断结果与术后病理对比分析。结果卵巢恶性肿瘤以Ⅲ型血管为主,卵巢良性肿瘤以Ⅰ、Ⅱ型血管为主,两者之间差异具有显著性(P<0.05)。三维彩色多普勒超声诊断卵巢恶性肿瘤的灵敏性、特异性、阳性预测值、阴性预测值分别为95.7%,80%,88%,92.3%,89.5%.结论三维彩色多普勒超声可客观地评价卵巢肿瘤的血管,以二维形态学为基础结合三维彩色多普勒超声综合评价可提高卵巢恶性肿瘤的早期检出率。三维彩色多普勒超声能明确地定位病变的位置与范围以及与临近组织结构的毗邻关系等,对二维超声做出有益的补充,对妇科手术方案的制定具有指导意义,具有比较广泛的临床应用前景。
