摘要
目的调查现阶段北京市急性脑卒中患者的院前延迟时间以及急救系统对院前延迟时间的影响,并对北京市急性脑卒中院前救治系统的建设提出初步的建议。方法收集北京市现阶段急性脑卒中患者的就诊流程资料,详细记录院前延迟时间段以及患者的一般信息,总结影响急救系统院前延迟时间的主要因素。结果2005年10月—2006年10月,共收集1029例患者的资料,其中使用急救系统(EMS)的院前延迟中位时间为4.65h,未使用EMS的院前延迟中位时间为6.95h,两组间差别有统计学意义(P=0.012);单因素分析中可见受教育程度、基层医院延迟、EMS使用、居住情况、疾病严重程度5个因素对发病至决定就诊时间的影响有统计学意义(P<0.05)。多元Logisic回归模型显示影响因素主要有发病时间、基层医院延迟以及发病后的疾病严重程度,EMS的因素未进入方程。结论北京市急救系统在一定程度上可以缩短急性脑卒中患者的院前延迟时间;应当进一步加强公众脑血管病知识教育,宣传EMS的作用,同时加强医院与急救系统的密切合作,使急救系统能够在急性脑卒中患者的院前急救环节发挥更大的作用。
