糖尿病病人围手术期处理

Perioperative Management of Patients with Diabetes Mellitus

目的 :明确糖尿病病人围手术期血糖控制的安全措施。方法 :对 38例合并糖尿病的外科病人 (血糖大于8.1mmol/L)围手术期处理进行了回顾性分析。结果 :发现 2例病人病情隐匿 ,术前漏诊 ,在术后发生严重并发症才明确诊断。在治疗中 3例出现Somogyi效应 ,1例出现对胰岛素拮抗 ,有 3例发生了糖尿病危象。 结论

Objective: To find out a safe method of peri operative glycemic control in diabetic patients. Methods: A retrospective analysis was made on perioperative management of 38 surgical pat ients with diabetes. Results: 2 patients were misdiagnosed due to insidious diabetic course. 3 patients developed Somogyi' s e ffect, antagonism of insulin appeared in 1 patient and 3 patients occurred the crisis of diabetes mellitus. Conclusion: The proper us e of insulin is an effective and safe method for the management of diabetic pati ents during the perioperative period.
