摘要
目的:探讨重度胸部创伤的早期诊断与治疗。方法:回顾性分析78例重度胸外伤患者的临床资料。结果:本组ISS评分17分～25分49例,≥25分29例,治愈64例,死亡14例。闭合伤死亡10例,开放伤死亡4例。其中因急性呼吸窘迫综合征(ARDS)死亡5例,失血性休克3例,弥散性血管内凝血(DIC)和心脏大血管破例各2例,颅内出血并脑疝和多器官功能衰竭综合征(MODS)各1例。结论:严重胸部创伤多发伤和并发症多见,表现复杂,死亡率高,早期全面正确的诊断与及时有效的处理是减少死亡的关键。
Objective:To explore the early diagnosis and treatment of severe thoracic trauma.Methods:The clinic data of 78 patients with severe thoracic trauma were analyzed retrospectively.Results:ISS scoring in 49 cases was between 17 to 25 points,and 29 cases with the scoring over 25 points.64 cases recovered and 14 cases died. Ten cases died of closed trauma,4 cases of opened trauma,5 cases died with ARDS,3 with hemorrhagic shock,2 with DIC,1 with intracranial hematoma complicated brain hernia,2 with cardiac and large vessel rupture,1 with MODS.Conclusion:Severe thoracic trauma is associated with multiple injuries and complications.In an effort to reduce the mortality of patient with severe thoracic trauma,it is important to early diagnosis and effective treatment.
