绝对不应期电刺激对正常兔在体心脏心功能的影响

Effects of electric stimulations applied during absolute refractory period on cardiac function of normal rabbits

摘要 目的 观察绝对不应期电刺激对正常兔在体心脏心功能的影响并评估其安全性.方法 20只正常新西兰兔随机分成左心室前壁(10只)和室间隔(10只)两组.麻醉开胸后,分别于左心室前壁和经右心室室间隔放置一对刺激电极,通过心电的R波触发,于心肌兴奋的绝对不应期发放方波电流刺激(称为CCM).观察左心室收缩压(LVSP)、+dP/dtmax、心率、Q-T间期的变化以及恶性心律失常的发生率.结果 左心室前壁组CCM(10ms,10 V)刺激时,LVSP及其+dP/dtmax比刺激前分别增加了16.1%和19.8%(P<0.05),室间隔组CCM(10ms,10V)刺激时,LVSP及其+dP/dtmax比刺激前分别增加了9.2%和10.6%(P<0.05),且CCM对心率及Q-T间期无显著影响(P>0.05).持续15 min的CCM刺激未见恶性心律失常的发生.结论 于绝对不应期发放足够能量的电刺激能明显增强左心室的收缩功能,且安全性好,提示该方法是一种新的有效的调节心肌收缩的途径,为心功能衰竭的电刺激治疗提供了又一全新的途径.
