经中心静脉途径注入腺病毒转载的IκB基因对多器官功能障碍综合征大鼠Fas/FasL的调控作用

摘要 目的建立大鼠多器官功能障碍综合征(MODS)模型,将腺病毒转载的IκB基因导入大鼠体内,以了解其对MODS凋亡的调控作用。方法50只大鼠随机分为5组:A组(正常对照组);B组(MODS损伤1d组);C组(MODS损伤7d组);D组(腺病毒转载IκB治疗1d组);E组(腺病毒转载IκB治疗7d组)。观察5组大鼠从致伤开始后1、7d的各脏器功能的生化指标,大鼠肺、肝组织Fas/FasL表达。结果E组大鼠存活率与C组比较差异有统计学意义(P<0.05)。B、C组血肌酐、丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与A组比较差异均有统计学意义(P<0.05);B组血肌酐、丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与C组相比差异均无统计学意义(P>0.05)。D、E组血肌酐、丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与A组比较差异无统计学意义(P>0.05);D组丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与B组比较差异有统计学意义(P<0.05),而血肌酐水平差异无统计学意义(P>0.05);E组血肌酐、丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与C组比较差异有统计学意义(P<0.05);D组血肌酐、丙氨酸氨基转移酶、总胆红素、肌酸磷酸激酶水平与E组比较差异均无统计学意义(P>0.05)。5组大鼠pH值、PaO2水平比较差异均有统计学意义(P<0.05)。B、C、D、E组PaO2与A组比较差异均有统计学意义(P<0.05);E组PaO2与C组比较差异亦有统计学意义(P<0.05),D组与B组、B组与C组、D组与E组相比PaO2水平间差异均无统计学意义(P>0.05)。pH值:C组与A、B组相比差异均有统计学意义(P<0.05),其余各组相比差异均无统计学意义(P>0.05);5组大鼠PaCO2水平间差异无统计学意义(P>0.05,)。A组大鼠肺、肝组织中可见Fas蛋白表达,B、C组肺、肝组织中Fas蛋白表达较A组明显升高,以肺组织升高幅度最大。D、E组肺、肝组织中Fas表达较B、C组明显下降。A组大鼠肺、肝组织中可见FasL蛋白表达,B、C组肺、肝组织中FasL蛋白表达较A组明显升高,以肺组织升高幅度最大。D、E组肺、肝组织中FasL表达较B、C组明显下降。结论通过中心静脉途径注入腺病毒转载的IκB基因,可明显降低肝、肺组织的Fas和FasL表达,减轻MODS大鼠器官组织过度的细胞凋亡,改善预后。 Objective To probe into IkB gene regulation on apoptosis of multiple organ dysfunction syndrome(MODS) by establishing MODS models through introducing adenovirus-transported IkB gene into rats.Methods Fifty pathogen-free rats were divided randomly into group A (control group),group B (MODS 24 h),group C (MODS 72 h),group D (Adenovirus-transported IκB therapy,1 d),E group(Adenovirus-transported IκB therapy,7 d).The biochemical indicators of organs and Fas/FasL expressions of rat lung and liver tissues were observed on day 1,7 after injury.Results Adenovirus-transported IκB gene through central veins reduced the levels of creatinine,alanine aminotransferase,total bilirubin,creatine phosphokinase in MODS rats,improved the level of partial pressure of arterial oxygen,and reduced the expression of Fas /FasL in lung or liver tissues.Conclusion Adenovirus-transported IκB gene through central veins can decrease the expression of Fas/FasL in lung and liver tissues,reduce excessive tissue apoptosis and improve the prognosis in MODS rats.
