外伤性脑脊液漏治疗体位的探讨

Research into Treatment Posture in Leakage of Traumatic Cerebrospinal Fluid

目的探讨治疗外伤性脑脊液漏时采取个体化自由体位的可行性。方法 87例外伤性脑脊液漏随机分成平卧位头偏向健侧及半卧位头偏向患侧两组,观察研究两种体位对外伤性脑脊液漏愈合的不同影响。结果平卧位头偏向健侧组脑脊液漏停止时间为11小时～13天,平均3.3天;半卧位头偏向患侧组脑脊液漏停止时间为23小时～5天,平均3.6天。无1例颅内感染和再漏。两组愈合率及愈合时间比较无显著性差异。结论外伤性脑脊液漏时采取个体化体位不影响治疗效果,不增加并发症,且可减轻病人不良反应,提高其舒适度。

Objective To conduct research into feasibility of individual free posture in treatment of leakage of traumatic cerebrospinal fluid,Methods Eighty-seven cases of leakage of traumatic cerebrospinal fluid were ran- domly divided into two groups,The patient lay in a horizontal position with the head inclining to the healthy side and the patient lay in a semireclining position with the head inclining to the diseased side to observe the influence of two positions on healing of leakage of traumatic cerebrospinal fluid,Results The stop time of leakage of cerebrospinal fluid in patients who lay in a horizontal position with the head inclining to the healthy side was 11 hours～13 days (average 3.3 days),The stop time in patients who lay in a semireclining position with the head inclining to the dis- eased side was 3 hours～5 days(average 3.6 days),There was no one case of intracranial infection and releakage. There was no obvious difference in healing rate and time in both groups,Conclusion In leakage of traumatic ce- rebrospinal fluid, the individual position will not influence the curative effect,It can also relieve the patients' adverse reaction, add comfort, and avoid complications.
