Analysis of the clinical characteristics of retinal degeneration in myopia and relative therapeutics before LASIK operation

摘要 目的探讨LASIK术前筛查视网膜病变的临床特点及治疗方法。方法回顾性分析LASIK术前135例163眼近视性屈光不正视网膜病变的特点及多波长氪激光治疗的临床资料。结果近视性屈光不正视网膜病变的屈光度分布为:≤-3.00D,3眼,占1.84%,-3.00D~-6.00D,21眼,占12.88%,≥-6.00D,139眼,占85.28%。病变主要部位为:单象限占77.91%。病变的性质以格子样变性为主,占65.03%。病变范围多集中于一个象限。双眼共存病变28例,病变对称性分布21眼。光凝治疗后三月行LASIK手术,术后随访1518个月,无一例发生视网膜脱离。结论近视性屈光不正的视网膜病变分布为:85.28%为高度屈光不正。77.91%变性区域为一个象限,65.03%病变为格子样变性,双眼发病者75%病变对称分布。LASIK术前,近视性屈光不正的视网膜病变激光治疗可获得确切疗效。 Objective To investigate the clinical characteristics of retinal degeneration in myopia before LASIK operation and the therapeutic effect of Krypton ion laser photocoagulation. Methods To analyze the characteristics of retinal degeneration in 135 myopia cases(163eyes) and the clinical data of treatment by Krypton ion laser before LASIK operation.Results Among 163 eyes with retinal degeneration: ≤-3.00D, 3 eyes(1.84%) ;-3.00D~-6.00D, 21eyes(21.88%); ≥-6.00D ,139eyes(85.28). 77.91% of the cases had degeneration less than one quadrant. And the major type of the retinal degeneration is Grid-like degeneration. Retinal degeneration occurred in both eyes in 28 patients and 21 of whom were detected symmetry between both eyes. LASIK surgery was performed three months after laser photocoagulation. All cases were followed up for 15-18 months. No retinal detachment was found.Conclusion Among the patients with retinal degeneration in myopia, 85.28% were high myopia., 77.91% had degeneration less than one quadrant, and 60.03% were Grid-like degeneration. In patients with retinal degeneration occurred in both eyes, 75% were found symmetry between both eyes. The effect of Krypton ion laser photocoagulation before LASIK operation on the retinal degeneration in myopia is confirmed.
